How to register for life support services?

1. Get the Life Support form from either online or via post:
2. Download and fill in the Life Support Form from our website: https://www.covau.com.au/Forms
3. Phone us on 1300 689 866 and we’ll post a form out to you.
4. Complete the form, including the patient information, account holder details and, get the medical report completed and signed by a registered medical practitioner.
5. Return application via post or email:
